2015 IRS Verification of Non-Filing Letter

The Verification of Non-Filing Letter is a federal financial aid requirement that provides proof that the IRS has no record of a filed Form 1040, 1040A or 1040EZ for individuals who are not required to file.

Amended Tax Return

Please provide a signed copy of your Amended Form 1040X Federal Tax Return.
Asset Verification Form

FAFSA has requested that we verify your assets. Please complete the Asset Verification Form in order for us to proceed with processing your FAFSA.
Bankruptcy Status Letter

Due to bankruptcy, you must provide a letter from your attorney that you are currently authorized to assume additional debt.
Birth Certificate

Please provide a copy of your birth certificate in order to verify your date of birth.
Child Support Paid

Please provide proof of the total amount of child support paid for the previous year.
Confirm Degree Status

You indicated that you will have your bachelor’s degree by July of 2015. If this is in error please provide a written and signed statement indicating that you will not be receiving your bachelor’s degree by July 2015. Please include your name, student ID number, and social security number in your statement.
Copy of INS Alien Registration Documentation

In order to verify that you are an eligible non-citizen for purposes of receiving federal student aid, we must collect a copy of your INS Alien Registration documentation.
Copy of Marriage License

In order to verify the marital status reported on your FAFSA, please provide a copy of your marriage license.
DD-214 Form (Military Discharge)

You reported on your FAFSA that you are a Veteran of the US Armed Forces. In order to verify your status, we must collect a copy of your DD-214.
Death Certificate

In order to verify that your parent/spouse is deceased, please provide a copy of their death certificate so that we may exclude their information from your FAFSA.
Default Resolution Documentation

Please provide a letter from your student loan servicer(s) indicating you are no longer in default on your loan(s).
Dependent Child’s Birth Certificate

Please provide a copy of your dependent child’s birth certificate.
Drug Conviction Resolution

Your reported a drug conviction on your FAFSA. To resolve this status, please contact the U.S.D.E. at (800) 433-3243.
Emancipated Minor Documentation

You indicated on your FAFSA that you are an Emancipated Minor.  Please provide a copy of a court’s decision that as of today you are an emancipated minor or that you were an emancipated minor immediately before you reached the age of being an adult in your state. The court must be located in your state of legal residence at the time the court’s decision was issued.
FAFSA Discrepancy must be Resolved

There was discrepant information reported on your FAFSA that must be resolved. Please contact the Office of Financial Aid to determine what documentation is needed to process your application.
Foster Care Documentation

You indicated on your FAFSA that you are/were in Foster Care. Please provide a copy of the court documentation that verifies your status.
High School Diploma

Please provide a copy of your high school diploma or your final and sealed high school transcript to confirm your graduation status.
Orphan or Ward of the Court Documentation

You reported on your FAFSA that you are/were an orphan or a ward of the court. In order to verify your status, we must collect a copy of the court documentation specific to your situation.
Overpayment Resolution Documentation

The National Student Loan Data System indicates that you have received one or more overpayments of Federal Student Aid. You are not eligible to receive federal student aid until you resolve the overpayment. Please provide documentation from the appropriate agency verifying that your overpayment has been resolved.
Parent’s W2 Form(s)

Please provide a copy of your parent(s) W2 form(s) from the previous year (most current) in order to verify the working wages reported on your FAFSA.
Proof of Child Support Received

Please provide proof of the total amount of child support received during the previous year.
Proof of Citizenship

You must provide documentation to verify your citizenship status. Acceptable documentation includes a Certificate of Naturalization and/or a valid U.S. passport.
Social Security Card

In order to verify your social security number matches the name reported to the Social Security Administration, please provide a copy of your social security card.
Spouse’s IRS Tax Return Transcript

Please provide a copy of your Spouse’s IRS Tax Return Transcript. Transcripts can be downloaded from the IRS website.
Statement of Educational Purpose and Proof of Identity

In order to verify your educational purpose and identity, you can download the Statement of Educational Purpose and Proof of Identity form and return it to the Office of Financial Aid in person along with a picture ID.
Statement of Support Form

The income information you have provided on your FAFSA for your household is unusually low. In order to verify how your family was supported during the previous tax year, you must complete the Statement of Support form. Do not leave anything blank.
Student/Spouse W2 Form(s)

In order to verify your working wages, we must collect a copy of you and/or your spouse’s (if applicable) most recent W2 form(s).
Transcript Database View - TRDBV

If you are selected for verification and are unable to obtain a tax transcript due to being a victim of tax identity theft please contact the IRS Identity Protection Specialized Unit at (800) 908-4490 to request a copy of your Transcript Database View (TRDBV).
Verification of Legal Guardianship

You indicated on your FAFSA that you are or were in legal guardianship as determined by a court in your state of legal residence, prior to reaching the age of being an adult in your state. Provide a copy of the court’s decision documenting you are in legal guardianship from the state of your legal residence.
Verification of Second Degree

Your degree status is currently under review. In order to verify your status, please confirm whether or not you have previously obtained a bachelor’s degree.
Verification of Selective Service Registration

To receive federal student aid, you must be registered with Selective Service if you are male and between the age of 18 and 25. By answering ‘Register me’ on the FAFSA, you give the Selective Service permission to register you or you may register online at the Selective Service System and provide a copy to the Office of Financial Aid.
Verification of Sibling in College

You reported on your FAFSA that more than one family member is attending college. In order to verify this status, please provide an enrollment verification letter to confirm this status.
Verification of Unaccompanied Youth Status

You indicated on your FAFSA that you are an unaccompanied youth who is homeless or in danger of being homeless. Your situation must be documented from a homeless youth liaison, Runaway and Homeless Youth Act (RHYA) provider, or U.S. Department of Housing and Urban Development (HUD) provider.
Victims of Federal Tax Identity Theft Verification Statement

If you are selected for verification and are unable to obtain a tax transcript due to being a victim of tax identity theft please complete the attached form.
Unaccompanied Youth Documentation

You indicated on your FAFSA that you have been determined by the director of an emergency shelter or transitional housing program funded by the U.S. Department of Housing and Urban Development to be an unaccompanied youth who is homeless.

Homeless means lacking fixed, regular and adequate housing, which includes living in shelters, motels, cars, or parks, or temporarily living with other people because the student had nowhere else to go, including situations in which the student is fleeing an abusive parent who would otherwise provide the student with financial support and a place to live.

Unaccompanied means the student is not living in the physical custody of his/her parent or guardian.

Youth means the student is 21 years of age or younger or the student is still enrolled in high school as of the day he or she signs the application.

Please provide a letter from your homeless youth liaison verifying this status.
